Why does the company collect Personal Information?
The company use your personal information to process and review your laboratory test request so we can provide you efficient laboratory services, and to conduct studies and research for the purpose of improving and developing our existing services to you.
What type of personal information do we collect from you?
Personal information which includes your real name (last name, first name, middle name, date of birth, gender, mobile number, e-mail address and valid ID presented) that would allow us to contact you incase you have some concern and inquiry.
Why do we use your personal information?
The company will use your personal information to perform your laboratory test request and we will also use your personal information for reporting, analysis, survey and etc.
With whom may the company share the data?
The company will not share your personal information with third parties without your permission and this will only used for internal purposes only.
How does the company protect the data?
The Company strictly enforces its privacy policy. It has implemented the three privacy and security risk control measure (technological, organizational and physical) to protect personal information from loss, misuse, unauthorized modification, accidental access, change or destruction.
The Company uses safety measure such as the following:
• Totally no access to unauthorized personnel or visitors.
• Use of secured servers and firewalls, data encryption on computing devices, Antivirus software and ransomwares.
• Restricted access only for qualified authorized personnel.
• Strict implementation of company information security procedures.
Where and how long the company keep the personal information?
The Company stores personal information in both local and off-shore facilities, such as data centers (on premise and cloud) and physical documents are kept in secured storage facilities. Personal data collected are retained for five (5) years counted from the date you provide it to Singapore Diagnostics. The data can be deleted earlier if requested. Hard copies subject for deletion will be shredded before disposal.
What are the rights of data subjects under the Data Privacy Act?
In accordance with the Data Privacy Act, as a data subject, you have the right to be informed that your personal information will be, are being, or were, collected and processed, you also have a right to access copy of your personal information relating to you that they have on their computer database and/or manual filing system. You can exercise your right to object if the personal data processing involved is based on consent or on legitimate interest. You also have the right to suspend, withdraw or order the blocking, removal or destruction of your personal information. However, choosing not to disclose or deciding to remove your personal information from purposes. our systems may limit our ability to send marketing material and update laboratory test prices and services to you.
